What should I bring to my first appointment?

Please make sure that you arrive to our office at least 15 minutes before your scheduled appointment time so that you can provide us the necessary paperwork and insurance information to put you into our system. To save time, you can also print the New Patient Forms located on our forms page and bring them filled out to your appointment.

Do I need a referral to make an appointment?

No, you do not need a referral to make an appointment with either Doctor Graf or Doctor Harris.  They are both currently accepting new patients.  However, for your visit to be covered through your insurance please contact your insurance provider to understand the terms of your policy.

How is the doctor-patient relationship different here, compared to other offices?

This practice was founded on core values that encompass our initiative to provide patient-focused care. We take the time to listen to patients’ concerns and address any questions they may have.  We also feel that it is just as important for our nursing and office staff to take an active role in helping to achieve our goals by treating each patient with dignity and respect.  We value all of our patients and appreciate the long-lasting relationships that we create with them.

How will I pay for my care?

If you have insurance, please bring in your card and we will either collect your co-pay or bill you through your insurance plan. If you are not insured, we require payment at the time of service.  We strive to have all aspects of our practice, including our professional fees, reflect our ethical and rational approach to your care. 

What is your cancellation policy?

We request that you kindly provide the courtesy of calling within 24 hours of your missed appointment so that we may reschedule your visit.  Please note that if you do not call or show up to your appointment, you will be charged a $50 fee. 

What if I need to get in touch with Dr. Graf or Dr. Harris?

We take pride in our doctor-patient relationships and schedule plenty of time for you to get all of your questions answered.  If you need to contact Dr. Graf or Dr. Harris in between appointments, please contact our main office to speak with one of our helpful staff members so that they may assist you in relaying your concerns.  If you have an urgent medical concern that needs to be addressed after regular business hours, please call the main office line and you will be connected with the answering service.  

How do you notify your patients of lab or test results?

On the Patient Information Form that you fill out on your first visit, there is an opportunity to answer how you would like to be notified regarding any test or lab results. We are usually able to notify patients within 24-48 hours of receiving results.

Do you allow urgent or same day appointments?

We do set aside time for urgent visits. However, if you are having a life-threatening emergency, please dial 9-1-1.

What are your office hours of operation?

We are open Monday through Friday from 8:00am to 5:00pm and are closed each day between 12:00pm and 1:00pm for lunch.
